If you are Porting Voice Services for your account to a new service provider, the winning carrier must submit the request to Port those Services.

The winning carrier must submit those disconnect requests as the final step in their port-out request to Lumen based on their respective SPID (service provider identification).

Please DO NOT fill out a disconnect form for those services, as you may experience out-of-service issues.

**Please note: Only Voice Services will port out to a new carrier. If you have data services associated with the account that you would like to disconnect, a disconnect request will need to be submitted for those Data Services, click the button below to return to the main form and select the All Services or Specific Services disconnect option to complete your disconnection request.
